The purpose of this research survey is to continue our efforts in collecting data on Wake County's residents' experiences with COVID-19. In 2021, we heard the voices from residents across wake county about the lack of information and resources they received on Covid-19. Now a year later as Covid-19 deaths and infections have significantly decreased we are asking residents to share their perspectives on Covid-19 and what work still needs to be done. This is a monthly survey and all participation is optional. Upon completion of the survey, Wake County residents can be entered into a weekly drawing for $50. Only Wake county residents are eligible for the weekly drawing prize. NC Counts Coalition thanks you for your time and feedback in completing this questionnaire.
This is a research survey conducted by Health LIT4Wake through NC Counts Coalition based in Wake County, NC.
You are invited to participate in this research project because you are known as a resident of Wake County. Your participation in this research study is voluntary. You may choose not to participate. If you decide to participate in this research survey, you may withdraw at any time by simply not replying. If you choose not to participate in this study or if you withdraw from participating at any time, you will not be penalized.
The procedure involves filling out a text survey that will take approximately 5-7 minutes to complete. Your responses will be confidential and we do not collect identifying information such as your name, email address, or IP address.
We will do our best to keep your information confidential. All data is stored in a password-protected electronic format. To help protect your confidentiality, the surveys will not contain information that will personally identify you. The results of this study will be used for scholarly purposes only and may be shared with Health LIT4Wake representatives and stakeholders.
